The cotinine (nicotine) test does not detect secondhand smoke
The detection limit for a cotinine (nicotine) of 100 ng/ml is high enough to conclude that a positive result is indeed due to direct use of tobacco products rather than secondhand smoke. The cotinine level in a person exposed to “secondhand smoke” (a person who is surrounded by smokers and exposed to cigarette smoke without smoking themselves), even for those who smoke very rarely, is in the range of 10–30 ng/ml. With prolonged smoking—for example, several cigarettes a day—and in heavy smokers, the cotinine level can exceed 500 ng/ml.
